Is Doctors’ Dispensing better than Pharmacist Dispensing in serving the peoples’ total healthcare in Malaysia ?

In this article, I as a humble small town General Practitioner, will attempt to provide credible grounds as to why doctors should preserve the prescription and dispensing duties in Malaysia, and also why Dispensing Separation is not without many pitfalls. I am going to divide our discussions into 5 major topics:

1.    Where do we stand now in ranking of national primary healthcares?

2.    Does DS save costs of total national healthcares in Malaysia?

3.    What are the other controversies around Dispensing Separation?

4.    Doctors in Malaysia are not alone is questioning Dispensing Separation. UK and US studies

5.    What we think is best for the healthcare of the country.

6.    The consumer groups that are against DS.

 What is happening now?
Dispensing by pharmacist is akin to the depiction here, where the horse is the doctor.

The Malaysian Pharmaceutical Society have been pushing hard for the dispensing of medications to be handled solely by the Pharmacists with the tacit approval of the authorities. Things have come to the fore lately over likelihood of Pharmacy Bill which  being tabled in the Parliament in April 2015; however this was recently denied by the MOH and MMC.


Dispensing Separation  has been hotly debated by both divides of the healthcare profession, ie. the doctors versus the pharmacists. More than 95% of the Pharmacist support it; while most of the doctors are against.  This article is about doctors’ point of view : why are we against it so vehemently.

The public may think that doctors are trying to protect their rice bowl; rightly so because in this critical issue is a matter of doctors survival, not commercial monopoly. However, beyond that point, there are more to the major issue than meet the eyes regarding the critical welfare in the health of the consumers. At the heart of the issue, is whether the citizens of the country will benefit from the system, or will it cause irreversible hardships for decades to come ! Is DS better that Doctors’ dispensing ?

In reference to the article “Docs making a fast bucks-2nd of January 2013”….A very Damaging blanket statement THAT PAINTS ALL THE DOCTORS IN MALAYSIA.

We are aghast and was very upset with this piece of reporting as it has actually generalized all doctors (Private GPs) as greedy and unscrupulous professionals and thus this blanket statement has somehow triggered an uproar in our medical community…yes, we do admit there is black sheep among us but which profession does not have black sheep among them? The statement from the Pharmaceutical source of info is a well-known fact and a prolonged practice in Malaysia where there is a bonus schemes for the goods if the clinics or the hospitals needs to buy more for the sake of reduction of cost either which they will relayed it to the intended target that is our patients and despite the fact that even with this bonus schemes given by the Pharma companies to the doctors, the prices of medications still was and is much cheaper at the Mega Chains Pharmacies.
 
The accusation that middleman and runners doing the sales from doctors to Pharmacies is true for certain tainted individuals in both groups of professionals and they only escalates the price war which somehow does benefits the savvy consumers…this ongoing practices of big Pharma companies giving bonus schemes or inducement as mention by a Mr Gan in your article has been going on for a long time and it has been accepted as a common normal practices for all who are involved. But NOT ALL DOCTORS and CLINICS are doing this and most of us GPs are buying only what is essential for our patients needs and we do purchase from the pharmacies as well instead of from the main Pharma companies.
The practices of bonus schemes are not limited to our medical profession dealings…it is widely practiced in all business of commercial retailing and if this bonus scheme or inducement is not allowed then there is no competition in the market .Imagine Hypermarkets with the same cost purchase with those in the old wet market..if that happens, there won’t be anymore hypermarkets and definitely there won’t be middleman and runners as well. 

We are not supporting these kinds of inducements if there is unfairness in trade or if it has been proven to be costlier to our patients as well to a clinic cost managements. With these bonus schemes trade deals, the ultimate winner will always be the supplier and the creator of the aforesaid medication or products and by having proper and no abuse of medical inducement, we hope to relay this reduced cost to our patients.

We feel that the person who has spoken out against this unfair trade deals are JEALOUS of what the doctors have been given all these while and there is a reason why we doctors here in Malaysia has been given this trade deals….We are allowed to dispense(after a proper diagnosis) while the Pharmacist can’t . Even with that said and done, some of the Pharmacist has been selling indiscriminately WITHOUT a doctor’s prescription to our unsuspecting people and patients. Yes, we agree that they do have more inherent trained Pharmacological knowledge but they DO NOT POSSESSED the skills of diseases diagnosis, identifications and the actual management. There are some unethical Pharmacist here in Malaysia who labeled themselves

as “doctors” equipped with an examination rooms and all the gadgets too with Blood test facilities or certain Pathology labs in tow to operate within their premises. Since when they have that medical knowledge to handle diagnosing and initiation of treatment? We have collected substantial amount of proof and evidence to back this allegations of their unethical practices.

Let’s get back to the topic of certain ‘unscrupulous’ doctors who has this trade deal supplying the pharmacies with the aforesaid inducement prices…this is a private agreement with their own pharmacist friends to help to share the cost of this drugs/medications and this was meant well for the patients. All this was seen as profit minded and greedy intent but they never looked behind the actual ploy that will eventually benefits all parties (except the sick ones who will have to fork out more if there is no such bonus schemes).The sharing of cost management between this doctors and pharmacist has been ongoing and all this business part of trade deals will be finally for the benefits of the community…..do think of them as what big hypermarkets are doing now for our rakyat in this current stiff competitive market world. If the person and the associations disapproves of such trade deals then they should bring this matter up to the consumer complaints levels for unfair trade practices and all these should then be applied to ALL commercial business…that will be fair and finally, that will also remove competition in the market hence the rakyats will suffers due to this fair trade exclusions. No more cheaper medications and everyone should be happy. That will be the end of free market trade.

ABUSE OF MEDICAL LEAVE: 'Lax' doctors would be investigated and their clinics raided GEORGE 
TOWN: EMPLOYERS can lodge a report with the Malaysian Medical Council on doctors who issue medical certificates (MCs) too easily without proper examination.

Malaysian Medical Association president Dr S.R. Manalan said if there was a valid complaint, an investigation could be conducted and the clinic could be raided.

"MCs should only be issued after a medical examination.

"The council will investigate and if proven guilty, the doctor can either be let off with a warning or face suspension."

Dr Manalan was responding to a query on recent reports of doctors who issued MCs without medical examinations and workers who abused their sick leave privileges by feigning illness to obtain MCs.

Malaysian Employers Federation executive director Shamsuddin Bardan had said that based on a recent survey, on the average, each employee took about nine days per year of medical leave, or up to four per cent of working time.

He had said the system was too lax and because doctors wanted to build a good relationship with their patients, some were too easy on the MCs.

Raja Muda of Perak Raja Dr Nazrin Shah had first broached MCs when he said the initial purpose of medical leave was being abused, with doctors selling MCs to those who were not sick.

Dr Manalan said many factors had to be considered before an MC was issued.

"For example, a driver cannot drive if he has an injured leg, but a receptionist with a similar problem would still be able to do her job and answer calls."

He said employees who sought treatment at clinics often complained of diarrhoea, headaches or backaches -- ailment that was difficult to determine unless a thorough examination was done. In such cases, he said the companies should refer their staff for a medical check-up.

"The welfare of the workers is the responsibility of the employer. But if employees fake their illnesses, a check-up would scare them and this can cut down on the medical leave they take."

On the other hand, University Malaya Medical Centre (UMMC) consultant psychiatric associate professor Dr Jesjeet Singh Gill said if an employee took a great number of days for medical leave there could be an underlying problem.

"They could be suffering from anxiety attacks, under stress, or going through depression.

"There could be a number of reasons. Employers should first send their staff for a full medical check-up and when medical reasons are eliminated, they should consider counselling."

With the cost of absenteeism at about RM100 per day and the additional costs of replacing absent workers, the total loss due to sick leave stands about RM9 billion, or one per cent of Malaysia's gross domestic product of RM850 billion.

Cuepacs president Datuk Omar Osman urged heads of government departments to probe cases of civil servants abusing their sick leave.

He said civil servants should be mindful of their action so as not to undermine the integrity of the public sector.

Our members is always the top most priority in everything we do....we aim not just to please but we also aimed to be the mediator among ourselves.....time and time again, we have witnessed, how many of our members stood in to give advises without being asked, giving encouragement to others without being forced, helping out willingly and full heartily .....

Proactive and spontaneous , love without borders and discrimination, work with each other without being forced upon and most importantly SHARE without Prejudice......This is what a good association wants from it's members and from it's selected committee members....ALL were chosen for a reason and all these traits are being seen now in MPCAM/MPCN........and we want all our members to know, the true membership of this UNITY is our members knowledge of being here as a not only a member but as a true recognized FORCE and VOICE of our noble profession.....

Over the years of silence and isolation and Over the years of being bullied and hassled, we remain strong even then when we are alone with no avenues to vent out our problems or a willing ear to listen and hear our plight and pleas....

NOW,we have....we have a new home which this house my brothers and sisters built...This home is our Command centre , Our News portal , our Place of solace and renewable energy to carry on with our daily struggles and to know that we are no longer ALONE........

This is what MPCN meant to me.....and it is precious beyond all known treasures in the world........our members....the true, pure, and sincere Brothers and Sisters of Medicine.......VIVA LA MPCN/ MPCAM!!!!!!!
